Surgical treatment of locally spread head and neck tumours is a difficult problem, as the extensive defects formed after tumour removal cannot be closed with local tissues. The use of free flaps on microvascular anastomoses allows full one-stage replacement of the defect of muscular and covering tissues.

However, the technique is only available in a few oncological centres. Flaps with axial blood supply and relocated regional flaps can be an alternative to microsurgical flaps.

The use of flaps on a pedicle for reconstruction of extensive postoperative defects allows to expand the indications for conservative surgeries in patients with locally spread tumours, significantly improving functional and cosmetic results of treatment and quality of life of patients. The monograph is devoted to surgical interventions in head and neck cancer.

Special attention is paid to the characteristics of post-resection defects, the choice of plastic material depending on the category of their complexity, localisation and stage of tumour process, immediate and distant results. The parameters of patients' quality of life before and after reconstructive interventions are also evaluated.
